- Weekly during work: Certified Payroll WH-347 with wet signature (prime + every sub, numbered sequentially) — Statement of Compliance (WH-347 page 2) — Section 3 Man Hour Report — “No Work” payroll submitted if no work
- Monthly / periodic: HUD Form 2516 (contract/subcontract report) — Section 3 hiring updates — MBE/WBE participation tracking
- Payment requests: AHS Payment Request Checklist attached to every draw
- Any wage classification not on WD: SF-1444 request to AHS/DOL (30-day response minimum)
- Any underpayment discovered: Restitution Affidavit + back-wage payment (interest per 26 U.S.C. 6621, compounded daily)

Scope of Work — 4-Phase Approach
Sequenced per Firm Foundations' proposed phasing in RFI #001, Item 10 — back pond & rock road first (opens access), then drainage & basins, then the north road, then the south road. Target duration ~3 months, exclusive of inspection wait times. All phases contingent on engineer's written response to RFI #001 & any addenda.
Phase 1 — Back Pond + Rock Road
- Install silt fencing & tree protection per C-003 (limits per RFI #4)
- Selective clearing & tree removal at Pond 2 / Building 3 corner (~12 trees pending RFI #3)
- Protect existing pump shed at top of bank (RFI #5 method TBD)
- Pond 2 edge-of-bank excavation & grading per C-100 / C-501
- Install rock road / turf-block per corrected detail (RFI #8)
- SWPPP inlet protection in place before soil disturbance
Phase 2 — Drainage & Basins
- Install RCP storm pipe per C-102 (conflicts w/ canopy, playground, sidewalk — RFI #6)
- Temporary removal & replacement of playground surfacing during pipe work
- Sidewalk restoration where pipe crosses (RFI #6 confirms inclusion)
- Catch basins, inlets, and structure grade rings
- Regrade around basins to positive drainage
- Materials testing round #1 (Owner-paid per RFI #11)
Phase 3 — North Road
- Subgrade evaluation — undercut / stabilize per RFI #2 findings
- Root pruning if required (tree fall responsibility per RFI #2)
- Import & compact limerock base per FDOT spec
- Prime coat + SP asphalt lift per plans
- Sidewalk ramps installed with corrected ADA striping (RFI #7)
Phase 4 — South Road
- Mirror of Phase 3 for the south segment
- Sidewalk repairs / replacements & ADA warning surfaces
- Final striping, wheel stops, ADA signage & signposts
- Install 6′ security fencing per corrected limits (RFI #12)
- SWPPP take-down and re-vegetation of disturbed areas
- Punch list, site walk with Owner's Rep & Engineer
- Certified payroll & Section 3 close-out reports to Owner
